About the Program
The Healthy Meals, Healthy Minds Grant Program for Family Caregivers supports primary
caregivers by providing funding to strengthen services and support for family caregivers.
The program allows caregivers to invest in their mental health and wellness and access
nutritious meal kits.
Step-by-Step Application Process
Complete the Eligibility Criteria Form
The family or primary caregiver, also known as the applicant, completes the
Eligibility Criteria Form.
Submit the Supporting Documentation Form
The ADRC or County submits the Supporting Documentation Form for RCAW Grant Programs.
RCAW Reviews the Forms
RCAW receives and reviews the Eligibility Criteria Form and Supporting Documentation Form.
Qualified candidates will be emailed a link to the HMHM application.
Complete the Application
The family or primary caregiver completes and submits the HMHM application.
RCAW will notify the applicant by email if the application is approved and will provide
a link to the Grant Report.
Receive Grant Funds
Approved applicants will receive the grant funds by USPS.
Important Requirements
- Applicants must use HMHM funds within 60 days of receiving them.
- Applicants must track expenses using the Grant Report.
- Applicants must keep receipts and invoices for services rendered and purchases made with the funds.
- Each quarter, RCAW will randomly select applicants for audit.
- If selected for audit and unable to provide a completed HMHM Grant Report with receipts
or invoices, the applicant will immediately become ineligible for future funds and may
have to repay RCAW for the grant funds.
