Families may be eligible for other programs that provide ongoing or more comprehensive support. These programs may better meet your needs depending on age, diagnosis, or eligibility. Select the option that best describes your situation to explore programs you may be eligible for:
I am caring for a child (birth–21) with a disability or complex needs: You may want to explore:
- Children’s Long-Term Care Support (CLTS)
- Provides Medicaid-funded supports and services for children with disabilities and complex needs.
- Children’s Community Options Program (CCOP)
- CCOP pays for many supports and services that you can’t get through other insurance, Medicaid, or the Children’s Long-Term Support Program. It can also pay for a one-time need.
- Care4Kids
- Care4Kids helps children and teens in foster care find the health services they need.
- Children with Medical Complexity Programs
- Available through hospitals in Madison, Marshfield, and Milwaukee.
Eligibility typically extends from birth through age 26, with some policy considerations for ages 21–26.

- Provides Medicaid coverage for children with long-term disabilities or complex medical needs, regardless of parental income.

I am caring for an older adult (60+) or someone with dementia:

- National Family Caregiver Support Program (NFCSP)
Supports caregivers of:
- Adults age 60+
- Individuals with Alzheimer’s or other dementias (any age)
- Grandparents/relatives age 55+ caring for children under 18
- Alzheimer’s Family Caregiver Support Program (AFCSP)
- Provides support for caregivers of individuals with irreversible dementia, including Alzheimer’s disease and related conditions.
